Confirmatory Action Letter from A. B. Davis to Cordell Reed (CAL RIII-88-12) dated May 18, 1988 Reference B.
Commonwealth Edfson letter from J. A. Silady to A. B. Davis dated May 24, 1988 In response to your finding that all eight main steam isolation valves (MSIVs) failed to respond. as expected during a special slow loss of operating air test, you developed a plan to determine the cause and corrective actions.
We discussed your proposed course of action and documented our und~rstanding in Reference A.
Subsequently, on May 19, 1988, we established an augmented investigation team (AIT) to monitor your program. and independently assess the cause and significance of the apparent failures.
On May 24, 1988, the AIT completed its assessment, conducted its exit meeting with you, and in a telephone conversation between E. Eenigenburg and M. Virgilio you delineated short-term and long-term planned actions to enhance the reliability of the MSIVs.
Reference B. documented these commitments.
We have reviewed your findings, conclusions and proposed corrective actions related to the MSIV operator response on a slo~ loss of ~otive air. Based on this review, we conclude that short-term actions will ensure that the 8806100233. 880~~5 PDR ADOCK 05005~37 P
