Appendix A NOTICE OF VIOLATION Commonwealth Edison Company Docket No. 50-237 As a result of the inspectio~ conducted on August 1 through September 4, 1981, and in accordance with the Interim Enforcement Policy, 45 FR 66754 (October 7, 1980), the following violatidn was. identified:
Technical Specification 6.2.A. required in part, detailed written procedures including applicable checkoff lists... shall be prepared, approved and adhered to for prevention and corrective maintenance operations which could have* an effect on the safety of the. facility.
Contrary to the above, during the Unit 2 refueling outage in January 1981',
the 2-2301-3 valve (HPCI Turbine Steam Supply) was disassembled and rebuilt by maintenance personnel without an appropriate procedure or reference information.
This resulted.in the valve disc being reinstalled improperly, which prevented condensate drainage from the steamline resulting in the HPCI system being inoperable.
This condition existed for several weeks of reactor operation until it was discovered, reported and corrected by the licensee on May 26, 1981.
This is a Severity Level IV violation (Supplement I).
Pursuant to the provisions of 10 CFR 2.201, you are required to submit to this office within thirty days of the date of this Notice a written statement or explanation in reply, including for each item of noncompliance:
(1) cor-rective action taken and the results achieved; (2) corrective action to be taken to avoid further noncompliance; and (3) the date when full compliance will be achieved.
Under the authority of Section 182 of the Atomic Energy Act of 1954, as amended, this response shall be submitted under oath or affirmation.
Consideration may be given to extending your response time for good cause shown.
