Dresden Nuclear Power Plant, Unit 3

NRC Inspection Report 50-249/97027(DRS)

This inspection included a review of the implementation of the inservice inspection program for

the Unit 3 refueling outage 14.

Maintenance:

In general, the licensee's inservice inspection program implementation met ASME,

Section XI requirements (Section M3).

A violation was cited for inability to retrieve the radiographic record of a Unit 3 pipe weld

(Section M3).

The inspector reviewed inservice examination reports and ISi personnel certifications for

examinations recorded in "Dresdefl Station D3R14 Refueling Outage lnservice

Inspection Summary Report." The examination reports reviewed included the following:

Observations and Findings

Personnel certification records met ASME Code requirements for the selected

examinations completed in refueling outage D3R14. The examination records were

complete with the appropriate calibration records and required examination data. For

the selected examinations reviewed from the summary reports, the inspector identified

no deviations from ASME Code requirements, e~pt as discussed below.

The inspector identified that the radiographic film of the examination of Unit 3 high

pressure coolant injection turbine drain pot return line extension pipe replacement weld

(work request 950065745, minor plant change E12-3-95-246), could not be retrieved by

the licensee. The licensee subsequently issued problem identification form 01997-

08032 to address this issue. The licensee's inability retrieve the radiographic record of

the Unit 3 HPCI pipe weld {FW-3) is considered a violation of 10 CFR 50, Appendix B,

Criterion XVII, "Quality Assurance Records," {VIO 50-249/97027-01{DRS)).

Conclusions

The inspectors determined that ASME Code requirements had been met for select

examinations completed in the refueling outage D3R14. However, the inspector

identified that the radiographic record of the Unit 3 pipe weld could not be retrieved.

Quality Assurance in Maintenance Activities

ISi Program Implementation

a.

Inspection Scope (73051)

The inspector reviewed the licensee's ISi program plan, "Dresdan Station 3rd Interval

lnservice Inspection Plan," effective March 1, 1992 through and including February 28,

2002.

b.

Observations and Findings

The plan represented the third ten-year interval of the ISi program for Dresden Station

Units 2 and 3. The licensee had procured the services of an Authorized Nuclear

lnservice Inspector (ANll) from Hartford Steam Boiler Inspection and Insurance Co. of

Hartford, Connecticut. The ANll reviewed procedures, personnel qualifications,

instrument and material certifications, and examination results. Examinations were

  • performed in accordance with the Unit 3 Technical Specifications, the ASME Code,

Section XI, 1989 Edition, and Generic Letter 88-01. The inspector's review indicated

that the ISi program was acceptable.

Conclusions

The licensee's ISi program implementation met ASME Code Section XI, 1989 Edition

requirements.

The inspector presented the inspection results to a member of licensee management at

the conclusion of the inspection on November 19, 1997. The licensee acknowledged

the findings presented and did not identify any of the potential report input discussed as

proprietary.
