10 CFR Part 50, Appendix 8, ~riterion XVI, states, *in part, that measures shall be established to assure that conditions adverse to quality, such as failures, malfunctions, deficiencies, deviations,.defeC:tive material and equipment, and nonconformances are promptly identified and corrected.* In the case of significant conditions adverse to quality, the *measures shall assure that the cause of the condition is determined and corrective acti.on taken to preclude repetition.

A; Contrary to the above, corrective actions to conditions adverse to quality occurring on May 9, 1992, and September 18, 1992, failures to follow radiation protection (RP) procedures bymechanical maintenance
