I During normal operation, the 2-1402-4B flow test valve failed to close fully against pump pressure while performing DOS 1400-1, core spray system pump operability.

The core spray pump was secured and the valve closed fully.

Safety significance was [QJ}J minimal since the valve is normally closed duringcore spray operation.

This event i o I 6 I occurred again on 10/23/78 and 10/24/78.
