Perry Nuclear Power Plant Docket No. 50-440, License No. NPF-58 NRC Post-Examination Documentation. Perry License Class 1LOT17 In accordance with the guidance provided in NUREG-1021, "Operator Licensing Examination Standards for Power Reactors" (Revision 10), ES-501, "Initial Post-Examination Activities" Section C.1, the following materials are submitted in support of the Perry Nuclear Power Plant written operator license examination conducted on February 16, 2017:
- 1. Graded written examinations (i.e., each applicant's original answer and examination cover sheets) plus a clean copy of each applicant's answer sheet (ES-403, "Grading Initial Site-Specific Written Examinations")
- 2. Master examination(s) and answer key(s}, annotated to indicate any changes made while administering and grading the examination(s) (ES-402, "Administering Initial Written Examinations," and ES-403)
- 3. Questions asked by and answers given to the applicants during the written examination (ES-402)
- 4. Substantive comments made by the applicants after the written examination, with an explanation concerning why the comment was accepted or rejected (this item is encouraged but not required) (ES-402)
- 5. Seating chart for the written examination (ES-402)
- 6. Completed Form ES-403-1, "Written Examination Grading Quality Checklist" (ES-403 and Section D.1)
- 7. Results of any performance analysis that was performed for the written examination, with recommended substantive changes (ES-403)

Attachment 6, ES-403-1 is located in the Administrative Files ADAMS Package Attachment 7 was reviewed, evaluated, and discarded.
Attachment 8, ES-201-3 "Examination Security Agreement" is located in the Administrative Files ADAMS Package Attachment 9 incorporated into the as-administered Admin JPM file in the Administered Exam ADAMS Package

- 8. Original Form(s) ES-201-3, "Examination Security Agreement," with a pre- and post-examination signature by every individual who had detailed knowledge of any part of the operating tests or written examination before they were administered
- 9. Original Form ES 301-1 (SRO) and JPM AS-5, Administrative Topics Outline, for a Substitution during the Exam The post examination performance analysis identified potential areas of weakness. Two questions on the Reactor Operator Exam (R0-24 and R0-45) and four questions on the Senior Reactor Operator Exam (SR0-03, SR0-09, SR0-22, and SR0-25) were missed by 50 percent or more of the applicants. This condition was entered into the Corrective Action Program under Condition Report 2017-01751 for further analysis.
