NOTICE OF VIOLATION AND NRC REGION I COMBINED INSPECTION
50-387/94-16;
50-388/94-17
The enclosed report documents
a resident safety inspection
conducted
by Mr. D.
J.
Mannai,
and others
from July 19,
1994 through September
5,
1994, at the
Susquehanna
Steam Electric Station
(SSES),
Salem Township,
Areas relevant to the health
and safety of the public are described
in the
enclosed report.
At the conclusion of the inspection,
Mr. D. J. Mannai,
Acting Senior Resident
Inspector,
summarized
the inspection findings with Mr.
H.
G. Stanley
and other members of youl staff.
These violations appear to identify recurring weaknesses
in the implementation
of your fire protection
program
and corrective action processes.
We also
consider that ineffective communications,
relative to management
expectations
for compensating for an inoperable fire protection
system,
was
a contributor
that should
be addressed
in your response
to this apparent violation.
You are required to respond to this letter and should follow the instructions
specified in the enclosed
Notice when preparing your response.
In your
response,
you should
document the specific actions taken
and
any additional
actions you plan to prevent recurrence.
After reviewing your response
to this
Notice, including your proposed corrective actions
and the results of future
inspections,
the
NRC will determine
whether further
NRC enforcement
action is
necessary
to ensure
compliance with NRC regulatory requirements.
In accordance
with 10 CFR 2.790 of the NRC's "Rules of Practice,"
a copy of
this letter
and its enclosures will be placed in the
NRC Public Document
Room.

Overall, your staff operated
both units safely
and demonstrated
a strong
safety perspective.
However,
based
on the results of this inspection, certain
of your attivities appeared
to be in violation of NRC requirements.
Specifically, our inspectors identified,
on two occasions,
that fire doors
had
been blocked
open without the required administrative authorization.
Though
your staff identified several
previous instances
in which other fire doors
had
been blocked
open without required administrative approval, your corrective
actions in this matter appear ineffective.
Further, following the lightning
strike to the Simplex fire protection system
on August 2,
1994, which
adversely
degraded fire detection
and suppression
capabilities,
your staff
failed to complete the implementation of the compensatory
measures,
as
specified in the Technical Specifications,
within the time allowed.
We note
that this matter is similar in nature to previous events involving the the
Simplex fire protection
system.
The responses
directed
by this letter and the enclosed
Notice are not subject
to the clearance
procedures
of the Office of Management
and Budget
as required
by the Paperwork Reduction Act of 1980, Public
Law No. 96.511.
