The enclosed
report documents
a routine safety inspection
conducted
by
inspectors
of this office on May 21,
1989 - June
24,
1989, at the Susquehanna
Steam Electric Station
(SSES),
Salem Township, Pennsylvania.
The inspection
covered plant operations,
surveillance,
and maintenance
for both units.
At the
conclusion of the inspection,
Mr. S. Barber
summarized
the inspection findings
to Mr. R.
Byram and other members of your staff.
Based
on the results of this inspection, it appears
that certain of your
activities were not in full. compliance with NRC rules
and regulations'one
I
and Zoril III ventilation systems
were inadvertantly crosstied
when
a wall
separating
the zones
was
removed for outage work.
This is
a recurring problem
which indicates that past corrective action
has not been effective.
Although
the likelihood of this event occurring in the near future is low since neither
unit is in a refueling outage,
the
NRC is concerned
that your proposed corrective,
which involves additional administrative controls,
may not provide you the
necessary
assurance
that this event will not again recur.
Plant modifications
should
be considered.
A response
to this violation is required
and should
address
your corrective actions
and other information specified in Appendix A.
