APPENDIX A NOTICE OF VIOLATION Pennsylvania Power and Light Company Susquehanna Unit 2 Docket No. 388 License No.

NPF-22 During an NRC inspection conducted on May 21, 1989 - June 24,

1989, one viola-tion of NRC requirements was identified.

In accordance with the "General Statement of Policy and Procedure for NRC Enforcement Actions",

10 CFR Part 2, Appendix C

(NRC Enforcement Policy), the violation is listed below:

Technical Specification 3.6.5. 1 requires that secondary containment integrity be maintained while in Operational Condition 1, 2, 3, and when irradiated fuel is being handled in secondary containment.

In addition, secondary containment penetrations are required to be closed or capable of being closed by an automatic isolation system per Technical Specifications 1.37 and 4.6.5. 1.b.3.

Contrary to the above, during the period May 11 - May 20, 1989 while in Operational Condition 1, secondary containment penetrations (manual dampers XD-17513 and XD-17514) between Ventilation Zones I and III, which were required to be closed, were not clo'sed or capable of being closed by anwutomatic isolation system.

Pursuant to the provisions of 10 CFR 2.201, Pennsylvania Power and Light is hereby required to submit to this office within 30 days of the date of the letter transmitting this Notice, a written statement or explanation in reply, including:

(1) the reason for the violation if admitted; (2) the corrective steps which have been taken and the results achieved; (3) the corrective steps which will be taken to'void further violations; and (4) the date when full compliance will be achieved.
