This refers
to the
routine safety
inspection
conducted
by inspectors
of this
office on January
1,
1989 - February
4,
1989, at the Susquehanna
Steam Electric
Station,
Salem
Township,
The
results
are
documented
in
the
enclosed
inspection
report.
The inspection
covered plant operations,
surveil-
lance
and
maintenance
activities for both units.
At the
conclusion
of
the
inspection,
Mr. F.
Young summarized
the inspection findings to Mr. R. Byram and
other members of your staff.
\\
Based
on the results
of this inspection, it appears
that certain plant evolu-
tions were not conducted
in full compliance with NRC requirements.
Of particu-
lar concern
were
the
circumstances
associated
with exceeding
the
allowable
cooldown rate after the reactor trip on January
12,
1989.
You did not identify
the excessive
cooldown rate until several
days later.
The failure of both the
shift crew
and your post-trip
review process
to recognize
the
cooldown rate
demonstrates
the
need for prompt evaluation
and corrective action,
especially
when
considered
collectively with several
other operating
errors that led to
recent
unplanned trips and
ESF actuations.
Although
we
are
not concerned
with your overall ability to operate
the plant
safely,
we view the
issues
associated
with these
events
to
be significant and
are
considering
further
enforcement
action.
Accordingly,
an
Enforcement
Conference
has
been
scheduled for 1:00 p.m.
on March 21,
1989, to discuss
these
matters with you in the
NRC Region I Office in King of Prussia,
During the Enforcement
Conference,
please
be prepared
to address
(1) the safety
significance
and root
cause
of the
event,
(2) broader
implications
of this
event
and other
operational
events,
and (3) your corrective
actions
taken or
planned,
including
measures
to
prevent
recurrence.
We will
forward
an
appropriate notification of enforcement action following the conference.
