This inspection

reviewed the circumstances

which led to the inoperability,

identified by your personnel,

of one loop of Emergency Service Water

(ESW) and

Residual

Heat

Removal

Service Water

(RHRSW) during the period April 4 -".April

21,

1985.

During this period Unit 2 was in Operational

Condition

1 andIwas

required

by your Technical Specifications

to have

two independent

operable

loops of ESW/RHRSW.

This condition was caused

by sliding links ("States" links) that were left open

during modification work performed

as part of, the Unit

1" Refueling Outage.

This condition existed for approximately four weeks,

although anomalies Iwere

observed

by the control

room operators

during the period.

We are concer'ned

that these

anomalies

were not aggressively

pursued

and that additional sliding

links were found to be

open in various

systems

in the plant.

This event

has

revealed

a need for increased

sensitivity to the broader

consequences

of

individual equipment anomalies,

particularly as it applies to the operability

of systems

and components

common to both units,

and for the strengthening

of

administrative controls.

Since this problem is, in our view, considered

to be serious,

we need

to',

schedule

an enforcement

conference

with you in the

NRC Region I office to

discuss

the underlying reasons

for this matter.

Please

be prepared

to diiscuss

plant modification work controls, post-modification testing, sliding link/

jumpers/bypass

controls,

drawing controls,

and Work Author'ization processing

procedures

which may have contributed to this event.

You should also

beI

prepared

to discuss

the corrective actions you have taken or plan to take.
