APPENDIX A NOTICE OF VIOLATION Pennsylvania Power

& Light Company Docket Nos.

50-387/50-388 Susquehanna Steam Electric Station Units 1 and 2

License Nos.

NPF-14/NPF-22 As a result of the Equipment Environmental Qualification inspection conducted on September 15-19, 1986 and in accordance with the NRC Enforcement Policy (10 CFR 2, Appendix C) the following violation was identified:

10 CFR 50, Appendix B, Criterion V requires that: "activities affecting quality shall be prescribed by documented instructions and shall be accomplished in accordance with these instructions."

Field Procedure No. FP-E-ll, revision 2, sheet 4 of 4 states:

"Ensure conduit entries into motor operators or entries to unscheduled T.B.'s for motor operators are sealed per E-49 paragraph 4.2. 15."

E-49, paragraph 4.2. 15 states that:

"Conduits located above or below shall be effectively sealed against dust, moisture and flax-soap with sealant material PR-895 (formerly PR-615) or PR-855 or equal."

Contrary to the above on September 16, 1986 Limitorque MOV No.

HV-2F008 was identified by the NRC inspector without the specified seal to prevent entry of

dust, moi sture and flax-soap from the flex-conduit into the Limitorque MOV switch compartment housing.

This is a Severity Level IV Violation.

Pursuant to the provisions of 10 CFR 2.201, Pennsylvania Power 5 Light Company of Allentown, Pennsylvania is hereby required to submit to this office within thirty days of the date of the letter which transmitted this Notice, a written statement or explanation in reply, including: (1) the corrective steps which have been taken and the results achieved; (2) corrective steps which will be taken to avoid further violations; and (3) the date when full compliance will be achieved.

Where good cause is shown, consideration will be given to extend-ing this response time.
