Notice of Violation ( Inspection Report No. 50-387/84-35)

This refers to the special NRC inspection conducted by the NRC Resident Inspec-tors on October 13-22, 1984, at the Susquehanna Steam Electric Station.

The report of the inspection was forwarded to you on November 15, 1984.

The in-spection was conducted to review the circumstances associated with the failure of four scram pilot solenoid valves during individual rod scram testing on October 6, 1984.

During the NRC review of that event which included previous scrams and surveillance data, a violation of a technical specification limiting condition for operation (LCO) action statement was identified.

On November 30,

1984, we held an enforcement conference with you and members of your staff during which the apparent violation, its causes, and your corrective actions were discussed.

The violation, which is described in the enclosed Notice, involved the failure to recognize, during a surveillance test conducted on June 25, 1984, that the average scram insertion times for the three fastest control rods in a particu-lar 2 x 2 control rod ar ray, exceeded its time limit for insertion from the "fully withdrawn" position for the first of four insertion positions.

Although the limits were not exceeded for insertion to the other three positions, inclu-ding the "fully'inserted" position, the violation is of concern because data existed to indicate the slow response time but the condition was not recognized.

The violation has been classified at Severity Level IV, in accordance with the NRC Enforcem'ent Policy, 10 CFR, Appendix C, as revised, 49 FR 8583 (March 8, 1984).

This severity level is based on consideration of the minimal safety significance of the actual sequence of events.

Nonetheless, these events demonstrate the need for more careful review of completed tests to assure that the potential for problems are identified in a timely 'manner and satisfactorily resolved.

After these events at Susquehanna, the specific cause of the problems with the solenoid valves was determined by the vendor/supplier (General Electric) to be degradation of a polyurethane disc.

The NRC is concerned that the supplier had previously replaced, for apparently unrelated

reasons, the polyurethane disc with Viton-A material in their spare parts kits for the solenoids, but the sup-plier did not inform you of this change.

Therefore, in your response to this letter and the enclosed Notice, you should also describe the actions that you will take to ensure that suppliers of materials to your facility inform you 8501140025 850104 PDR

of product improvements so that yo'u can evaluate the priority for implementing the improvement at your facility.

You are required to respond to the enclosed Notice and you should follow the instructions specified therein in preparing your response.

We will examine closely your corrective action and we emphasize that any similar violation in the future may result in additional enforcement action.

In accordance with 10 CFR 2.790(a),

a copy of this letter and the enclosures will be placed in the NRC Public Document Room.

The responses directed by this letter and the enclosed Notice are not subject to the clearance procedures of the Office of Management and Budget as required by the Paperwork Reduction Act of 1980, PL 96-511.
