NOTICE OF VIOLATION Pennsylvania Power and Light Company Susquehanna Steam Electric Station Unit 1 Docket No.
50-387 License No. NPF-14 EA No.84-134 On October 13-22,
- 1984, an NRC inspection was conducted to review the circum-stances associated with the failure of four scram pilot solenoid valves at Unit 1 during individual rod scram testing on October 6,
1984.
During the NRC review of this event and previous reactor scrams and surveillance tests, a vio-lation of a technical specification limiting condition for operation action statement was identified.
Specifically, on June 25,
- 1984, was used to satisfy certain surveillance requirements.
However, although the surveillance was signed by an individual and his supervisor as meeting all acceptance
- criteria, one of the criteria was not satisfied,
- namely, the average scram insertion time for the three fastest control rods in a particular 2 x 2 array from the "fully withdrawn" position to the first of four insertion positions, exceeded its specified limit.
Limits were not exceeded for movement of rods from the "fully withdrawn" position to the other three insertion positions, including the "fully inserted" position.
In accordance with the revised NRC Enforcement Policy (10 CFR Part 2, Appendix C), published in the Federal Register on Narch 8, 1984 (49 FR 8583),
the vio-lation is set forth below:

requires that with the reactor in operational conditions 1 or 2, the average scram insertion time, from the fully withdrawn position, for the three fastest control rods in each group of four control rods arranged in a two-by-two array, based on deenergization of the scram pilot valve sole-noids as time zero, shall not exceed any of the following:

Contrary to the above, on June 25,
- 1984, when Surveillance Procedure SR-155-003 was performed using scram insertion time data from a June 13, 1984 scram to demonstrate control rod operability in accordance with Technical Specification
- 3. 1.3.4.,
the average scram insertion time to notch position 45 for the three fastest rods in the four rod 2 x 2 array containing control rods 38-39, 38-43, 42-39 and 42-43, was 0.462 seconds; the control rods in the 2 x 2 array with the slower than average times were not declared inoperable; and the reactor was allowed to operate in Operational Condition 1 for various time intervals between June 25, 1984 and October 6, 1984 This is a Severity Level IV Violation (Supplement 1)
Pursuant to the provisions of 10 CFR 2.201, Pennsylvania Power and Light Company is hereby required to submit to this office, within thirty days of the date of the letter transmitting this Notice, a written statement of explanation in rep',y, including for each violation:
(1) corrective steps which have been taken and the results achieved; (2) corrective steps that will be taken to avoid further violations; and (3) the date when corrective actions will be completed.
Where good cause is shown, consideration will be given to extend the response time.
