This inspection
reviewed the circumstances
involved with the fai lure of four
scram pilot solenoid valves
(SPSV) during individual rod scram testing
on Unit
1 on October
6 and the subsequent
shutdown of Units
1 and
2 on October
13 due
to a deficient
SPSV disc material.
This inspection identified a problem in that, during the June
13,
1984 reactor
on Unit 1,
one
2 x 2 control rod array exceeded
the specified
insertion time limit.
This condition existed until
a subsequent
on July
3,
1984, although
a surveillance
conducted
on June
25,
1983,
using the June
13
data,
provided information by which the deficiency should
have
been identifieu.
Apparently,
due to inadequate
reviews of the completed surveillance
the defi-
ciency was not noted by your staff.
This is significant in that two of the
four control rods that failed to scram during October 6,
1984 rod scram
surveillance testing were in the
2 x 2 control rod array that exceeded
the,
specified
scram insertion time on June
13,
1984.
In order to further
understand
the circumstances
and,
most importantly, the
underlying reasons
for this occurrence
and to obtain information to assist
us
in determining the proper enforcement action,
we have
scheduled
an Enforcement
Conference
for Friday,
November 30,
1984.
The conference will begin at 2:00
p.m. in the main conference
room at the
USNRC Region I Office in King of
Prussia,
The acceptability of this meeting
schedule
has
been
confirmed with Mr.
W. Barbaric of your staff.
In addition to the subject mentioned
above,
we would also like to discuss
with you during the November
30 meeting the status
and results of those acti-
vities identified in Confirmatory Action Letter 84-18, dated October
17,
1984.
