APPENDIX A NOTICE OF VIOLATION Pennsylvania Power and Light Company Docket No. 50-387 This refers to the inspection conducted by a representative of the Region I (Philadelphia) office at the Susquehanna Steam Electric
- Station, Berwick, Pennsylvania, of activities authorized by NRC License No. CPPR-101.
During this inspection conducted
.on April 2-27,
- 1979, the following apparent.
items of noncompliance were identified.
a.
10 CFR 50, Appendix B, Criterion V, states, in part:
"Activi-ties affecting quality...shall be accomplished in accordance with these instructions, procedures or drawings...."
The Susquehanna PSAR Section D.2.5 states, in part:
"Activi-ties affecting quality will be prescribed by documented instruc-tions, procedures or drawings appropriate to the circumstances."
The Bechtel Quality Assurance Manual ASME Section III, Divi-sion 1, for Susquehanna Units 1 and 2, Appendix 3, Welding Standard WD-1, Revision 4, Section 7.4, Item (13) states, in part, "for each weld which requires a purge, the LWQCE (Lead Welding Quality Control Engineer) shall check before welding
- begins, to verify...that purging is being performed as required...
.and initial and date the space provided."
Contrary to the above, as of April 4,
- 1979, tack welding, in-tended to become part of the finished weld, had been completed for weld joint DCA-141-1, Field Weld Number 2, without the LWQCE, or his representative, checking that purging was being performed as required.
This item is an infraction.
b.
10 CFR 50, Appendix B, Criterion XIII, states, in part:
"Measures shall be established to control the...storage...and preservation of material and equipment in accordance with work and inspection instructions...."
The Susquehanna PSAR Section D.2.13 states, in part:
"The applicant's Quality Assurance Program will require written instructions and procedures for...storage...and preservation requirements."

Appendix A The General Electric Installation and Service Engineering Nuclear Quality Assurance Manual, Revision 0, Section 9.6.2,
- states, in part:
"The service manager shall provide written handling and storage procedures when required by the design specification."
Contrary to the above, as of April 25,
- 1979, no written instruc-tions had been provided to control the storage and maintenance of the Unit 1 Reactor Pressure Vessel.
This item is a deficiency.
