APPENDIX A NOTICE OF VIOLATION Pennsylvania Power and Light Company Docket Nos.
50-387 50-388 This refers to the inspection conducted by a regresentatjve of the Region I (Philadelphia) office at the Susquehanna Stehm Electric.Station, Berwick, Pennsylvania, of activities authorized by NRC License Nos.
CPPR-101 and CPPR -102.
During this inspection conducted on August 21-September 21, 1978, the following apparent items on noncompliance wer e identified.
These items are Infractions.
A.
10 CFR 50, Appendix B, criterion IX, states, in part:.
"Measures shall be established to assure that special processes, including welding,...are controlled and accomplished by qualified personnel..."
The Susquehanna PSAR Section D.2.9 states, in part:
"Personnel performing special processes will be qualified in accordance with documented procedures."
The General Electric Installation and. Service Engineering (GE I E
SE)
Nuclear guality Assurance Manual, Section 11.4.1 states, "All welders and welding operators shall have current welding qualifications for the welding procedure, they will be performing."
C'ontrary to the above, on August 30, 1978, welders who had completed jet. pump riser to brace arm welds in accordance with detailed welding procedure ISE-DNP-3004 dicL not have current, welding qualifications for that procedure.

B.
10 CFR 50, Appendix B, Critorion V, states, in part:
"Activities affecting quality...shall-be accomplished in accordance with these instructions, procedures, or drawings..."
The Susquehanna PSAR Section D.2.5 states, in part:
"Activities affecting quality will be prescribed by documented instructions, procedures or drawings appropriate to the circumstances."
Pennsylvania Power and Light Company The Dick Corporation guality Assurance Program Manual Section 5.1.1
- states, in part:
"Activities affecting quality shall be performed as prescribed by documented instructions, procedures or drawings..."
The'echtel Specification, 8856-C-36, governing the. concrete work for the Spray Pond, states, in Paragraph 7.4.9 (b):
Liquid mem-brane curing compound shall be Type 2 in accordance with ASTM C 309-58."
Contrary to the above, as of September 7, 1978, Type 1 curing compound had been used for curing concrete for the Spray Pond.
