This refers to the routine inspection
conducted
by Messrs.
P.
P. Narbut and
K. E.'ohnston
during the period of July 17 through September
3,
1988.
This
inspection
examined your activities
as authorized
by
NRC License
Nos.
and
At the conclusion of the inspection,
discussions
of their
findings were held with Mr. J.
D. Townsend,
and other members of your staff.
Areas
examined during this inspection are described
in the enclosed
inspection
report.
Mithin these
areas,
the inspection consisted of selective
examinations
of procedures
and representative
records,
interviews with
personnel,
and observations
by the inspectors.
No violations of NRC
requirements
were identified within the scope of this inspection.
However, the inspection findings did identify several
problems that should
be
addressed.
First, the problems identified in I&C procedure
backlog indicate
that
some corrective actions
have not been timely, such that (1)
a preventable
containment ventilation isolation event occurred
and (2) there is skepticism
among
I&C personnel
as to PG&E's ability to deal with problems, particularly
the guality Hotline.
Although good individual performance
was demonstrated,
that is,
an
I&C technician
avoided
a potential reactor trip; there
appears
to
be
a continuing problem which should
be addressed.

Error," as documented
in Paragraph"S=.d.
of the enclosure,
found that the root cause
analysis
and
corrective actions identified were considered
to be unacceptable.
This topic
has
been discussed
between your staff and the senior resident
inspector
on
September
12,
1988.
As a result of this conversation
a commitment was,made
to
provide
a revision to the
LER to clarify the root cause
analysis
and provide
more specific corrective actions.
Additionally, this is an unresolved .item
and your evaluation
as to the technical significance
should
be included in
your revision to the
LER.
Therefore, this letter
acknowledges
your commitme t

a timely response.

Finally, there were two trips during this inspection period (Paragraphs
4.i
and 4.j of the report) which could have
been prevented, if there
had been
appropriate prioritization and resolution of problems.
We understand
that you
have initiated a study of the problems,
and
we look forward to your
conclusions
and findings in this area.
Therefore,
we request that you provide your evaluation of these
conditions
and
your planned corrective actions within 45 days of the date of this letter.
In accordance
with 10 CFR 2.790 (a),
a copy of this letter and the enclosure
will be placed in the
NRC Public Document
Room.
