This refers to the
a routine inspection,
conducted
by Messrs.
P. P. Narbut,
M.
L. Padovan,
and K. E. Johnston
during the period of September
28 through
November 8,
1986.
This inspection
examined your act'ivities as authorized
by
NRC License Nos.
At the conclusion of the inspection,
discussions
of their findings were held with Mr. R.
C. Thornberry,
and other
members of your staff,
Areas
examined during this inspection are described
in the enclosed
inspection
report.
Within these
areas,
the inspection consisted of selective
examinations of procedures
and representative
records,
interviews with
personnel,
and observations
by the inspectors.
Based
on the results of this inspection, it appears
that, one of your
activities was not conducted in full compliance with NRC requirements,
as set
forth in the Notice of Violation, enclosed
herewith
as Appendix A.
Your
response
to this notice is to be submitted in accordance
with the provisions
of 10 CFR 2.201
as stated in Appendix A, Notice of Violation.
We are concerned
that, apparently,
an inattention to detail has resulted in
recent instances
of personnel error and/or problems with strict
implementation of approved procedures.
As a result,
we per'ceive that your
overall performance
has deteriorated.
Accordingly, in your response
to this
Notice of Violation, please
address
those actions
taken or planned to improve
this situation.
Additionally, another apparent violation of NRC requirements,
regarding
ineffective corrective action in dealing with the containment
personnel
airlock, is described in paragraph
6 of this report.
Enforcement actions
regarding this matter will be the subject of separate
correspondence.
In accordance
with 10 CFR 2.790 (a),
a copy of this letter and the enclosures
will be placed in, the
NRC Public Document
