NRC INSPECTION OF NINE MILEPOINT UNIT 2 EMERGENCY DIESEL
GENERATOR PISTONS AND CYLINDER LINER DISTRESS (NRC
INSPECTION REPORT 50-410/92-15)

This letter transmits the report for the inspection led by Mr. C. H. Woodard during the onsite
periods of May 12-14 and May 26-29,
1992.
This inspection assessed
the magnitude of the
emergency diesel generators (EDG) cylinder distress in both the Division 1 and Division II
units and the effectiveness of corrective actions.
At the conclusion of the inspection, an exit
meeting was held on May 29th with Mr. M. McCormick and other members of the plant
staff.
Overall, the results of the inspection indicate that plant management
was most responsive in
addressing
this potential deficiency which had been defined in a draft Cooper EDG users
group inspection guide dated April 8, 1992.
Although one of the two Cooper EDG units had
recently successfully completed five-year maintenance
and surveillance tests which met all
technical specification surveillance and maintenance
requirements
to qualify it for operability
and the other was near completion, each was successively
declared inoperable to perform the
draft guide inspection.
This inspection led to major EDG engine overhauls to replace
distressed
pistons and cylinder liners and to remove piston wrist pin seals and bottom skirt oil
scraper rings to iniprove lubrication.
The coordination of these inspections and replacements
with the EDG manufacturer and the users group was considered effective in providing for the
prompt identification of the damage and for the proper repairs and replacements
required to
restore the units.
The t'urther root cause evaluations and enhanced
EDG operational
inspections you plan should provide yourselves,
the industry, and NRC with additional
contirmations that the basic problems (root causes)
which led to this cylinder distress have
been corrected.
Your root cause analysis evaluation and enhanced
inspection findings will be
reviewed in future inspections.
