NRC Region I Combined Inspection Report No. 50-220/92-25 and

50-410/92-29

This refers to the results of the routine resident safety inspection conducted by Messrs.

W. Schmidt, W. Mattingly, S. Greenlee,

and P. Drysdale from November

1 through

December

12, 1992, at Nine Mile Point Units

1 and 2, Scriba, New York. Mr. Schmidt

discussed

the inspection findings with members of your staff at the exit meeting conducted on

December 22, 1992.

This inspection was directed toward areas important to public health and safety.

The

enclosed NRC Region I Inspection Report describes

the areas examined during the

inspection.

Within these areas,

the inspection consisted of observation of activities in

progress,

interviews with personnel,

and selective examinations of procedures

and

representative

records.

Both units were operated safely by your staff.

Excellent response

to a degrading condition in

the reactor protection system at Unit 2 and subsequent

excellent response

to the scram

condition were observed.

We do note that there was another partial loss of offsite power

caused by a failure of an offsite breaker.

The issue of control and maintenance of these

breakers remains a concern because of their potential effect on and challenges to safety-

related systems.

During observations of a surveillance test on the standby liquid control system we noted that

testing personnel did not notify the inservice testing department or prepare a procedure

change,

as required by the procedure,

when a test equipment gauge with a range outside of

that specified by the procedure was installed and used.

The safety significance of this

installation was minor, because

the gauge provided a digital read out.

During our review we

noted that the use of gauges with ranges outside the inservice testing plan requirements

has

been

a previous problem.

The failure to use appropriate equipment or make the appropriate

notifications is a violation
