NRC Inspection of Corrective Actions Taken Following February 21, 1992,
Nine Mile Point Unit 1 Inadvertent Isolation of the Ultimate Heat Sink
{NRC Inspection Report 50-220/92-81 and 50-410/92-80)
This letter transmits the report for the inspection led by Mr. J. E. Beall during the onsite
periods of February 29 - March 4, 1992, and April 10 - April 18, 1992. This inspection
assessed
effectiveness of corrective actions taken at both Units
1 and 2 following the
inadvertent isolation of Unit
1 from Lake Ontario, the unit's ultimate heat sink. At the
conclusion of this inspection, an exit meeting was held on April 18, 1992, with Messers,
J.
Firlit and M. McCormick.
Overall, the results of the inspection indicate that the short-term corrective actions were
effective in increasing worker sensitivity to shutdown risk and supervisor oversight of
ongoing maintenance.
Also, Niagara Mohawk internal assessments
were effective in
identifying weaknesses
and areas for improvement.
However, the long-term corrective
actions initiated to address the concerns were not yet complete and could not be assessed
for
effectiveness.
The corrective actions willbe reviewed in a future inspection.
