NRC Region I Augmented Inspection Team (AIT) Review of the

March 23, 1992, Nine Mile Point Unit 2 Loss of Offsite Power and Control

Room Annunciator

This letter transmits the AITreport for the inspection led by Mr. J. E. Beall between

March 23-28,*1992.

This inspection assessed

the circumstances,

causes,

personnel actions,

and the safety implications of the loss of offsite power and control room annunciators.

At

the con'elusion of this inspection, a public exit meeting was held on April 1, 1992, with you

and other members of your organization to discuss the preliminary findings of the AIT.

The areas examined during the inspection included matters important to protect the health

and safety of the public.

These areas are identified in the enclosed inspection report..

Within these areas,

the AIT conducted detailed examinations of the'breakers,

relays and

power supplies involved in the event and related indications in the control room; held

discussions

and formal interviews with personnel involved in this event; reviewed relevant

records including computer printouts before, during, and after the event, and trends of

pertinent plant parameters;

and evaluated the adequacy of established

procedures,

management

oversight, and personnel training.

The AITconcluded that the event was, initiated by an inadvertent technician performance

error caused,

in part, by human factors problems.

The event demonstrated

the vulnerability

'fthe current Nine Mile Point Unit 2 design to a loss of control room annunciators during

shutdown or transient conditions.

This vulnerability was increased by delays in correcting a

problem previously identified by your staff with one of the power supplies to the control

room annunciators.

Operator performance was acceptable but the AITidentified weaknesses

in management

support that hampered

the operators in responding to the loss of offsite power.

The trip of the Division IIIemeigency diesel generator demonstrated

a problem in a'ssuring

cooling water to the machine following a sequential, rather than instantaneous,

loss of offsite

power.

Mr. McCormack stated at the exit meeting that this problem would be addressed

prior to Unit 2 startup.

Also at the exit meeting, Mr. McCormack described

a number of

corrective actions in the areas of work control, personnel performance and equipment

reliability. I request that you respond to this letter within 30 days of your receipt of this

'eport.

Your response

should discuss your plan for corrective actions.

In accordance with 10 CFR 2.790 of the NRC's "Rules of Practice," a copy of this letter and

the enclosure willbe placed in the NRC Public Document Room.
