Ouring the week of September
18,
1989,
the
NRC administered requalification
re-examinations
to licensed operators
of Nine Mile Nuclear Station, Unit 2,
who had failed sections of requalification
exams given during the weeks of
July 17, July 24,
and July 31,
1989.
Examinations
were administered
in accord-
ance with Revision
5 to NUREG-1021,
"Operator Licensing
Examiner
Standards,"
specifically,
ES-601, "Administration of NRC Requalification
Program Evaluations."
As the purpose of the re-examination
was strictly to evaluate
operator
compe-
tency,
no formal re-evaluation
was
made of the facility's requalification
training program.
Another assessment
of the requalification
program will be
conducted at
a future date.
Until such time, the program is still judged
as
being unsati sfactory..=-
Generic findings and preliminary results of the operating
exams wire discussed
with you and your staff at the exit meeting held September
21,
1989.
NRC results indicate that two of two crews
passed
the simulator section of the
exam.
Additionally, six of six senior reactor operators
(SROs)
and six of
seven
reactor
operators
(ROs) passed all sections of their re-examination.
Section
4 of this report discusses
our review of your responses
to our concerns
with your initial Requalification Action Plan.
Generally, your responses
are
acceptable.
However, in responding
to
NRC Concern
8, you do not address
why
your Quality Assurance
Program,
an independent
assessment
method which is al-
ready in place,
was not
a factor in detecting
problems with your requalification
program.
You are requested
to review this question
and respond
to the
NRC
within 30 days of receipt of this letter.
The response
requested
by this letter is not subject to the clearance
procedures
of the Office of Management
and Budget as required
by the Paperwork Reduction
Act of 1980,
