During the weeks of July

17 and July 24,

1989,

the

NRC conducted

a requalifica-

tion program evaluation at the Nine Mile Point Nuclear Station, Unit No. 2.

The evaluation

was conducted

in accordance

with Revision

5 to NUREG-1021,

"Operator Licensing Examiner Standards,"

specifically,

ES-601,

"Administration

of NRC Requalification

Program Evaluations."

The evaluation

had two purposes:

(1) to evaluate

individual operator

competency,

and (2) to evaluate

the effect-

iveness of the licensed operator requalification training program at Nine Mile

Point, Unit 2.

Generic findings and preliminary results of the evaluation

were

discussed

during the interim and at the conclusion of the two week examination

period with members of your staff identified in the enclosed

report.

As a result. of, this evaluation,

the Nine Mile Point,. Unit'. 2 requalification

program has

been rated

as unsati sfactory.

NRC results indicate four of 12

rnactor operators

and five of 12 senior reactor operators: failed at. least

one..

portion of the examination.

Additionally, two of six crews were determined to

be unsatisfactory.

Your requalification

program

was determined to be unsatis-

factory based

on the total. number of individual failures",: in'addition, the

progra~ exhibited

numerous- weaknesses

which are discussed

in the enclosed

report.

As a partial justification for allowing continued operation of Nine Mile Point,

Unit 2,

an additional requalification examination

was conducted during the week

of July 31,

1989, for five additional individuals.

One of the

two senior

reactor operators failed the written portion of the examination.

All other

operators,

except

one reactor operator,

passed

the entire examination

and the

clew was determined to be satisfactory.

The one reactor operator did not take

the written examination but has

passed all other portions..

As was discussed

in a telapnone

conversation

between yourself and Robert Gallo

of this office on August 8,

1989, you are expected

to complete

remedial train-

ing for all operators

and all crews that failed the

NRC requalification exami-

nation.

Further,

the individuals who were failed by the

NRC will not be

. returned to licensed duties until they have

been successfully

re-examined

by

the

NRC.

Also, the crews that were determined to be unsatisfactory

shall not

be returned to licensed duties

as substantially the

same

crews until they have

been successfully

re-examined

by the

NRC.

The

NRC re-examination

is scheduled

for the week of September

18,

1989.

To support the

NRC re-examination,

you are

requested

to provide validated examination material to the

NRC for review and

approval

by September

5,

1989.

Section

6 of this report contains

the

NRC review of the Requalification

Program

Action Plan,

the Requalification

Exam Remediation

Schedule,

and the

NMPC Justi-

fication for Continued Operation

submitted to the

NRC on August 10,

1989.

Based

on our independent

assessment,

we agree that Nine Mile Point-, Unit 2,

may

continue

power operations for the reasons

stated

herein.

However,

we are

concerned with the adequacy of the action plan in that the deficiencies in the

operator training program are not fully addressed.

Specifically, the results

of the requalification examination

and the results of the- inspection

performed

on

BWR power osci llations indicate that performance

and knowledge deficiencies

exist in your operations

organization.

The action plan does not address

the

underlying reasons

why these deficiencies exist.

In addition, the plan

contains

no provisions for independent

assessment

of training program actions.

You are requested

to review the requalification program action plan

and respond

to

NRC concerns

by September ll, 1989.

The response

requested

by this letter is not subject to the clearance

proce-

dures of the Office of Management

and Budget

as required by the Paperwork

Reduction Act of 1980; PL-96-511.

In accordance

with 10 CFR 2.790 of the Commission regulations,

a copy of this

letter

and. the enclosure will be placed

in the

NRC Public: Document.i<oom.
