inspection
conducted

1989, at Nine Mile Point Units
1
and
2, Scriba,
A
summary of the
fi'ndings
was
presented
to
Mr. J . Willis and
other
members
of
your staff at the conclusion of the inspection.
One violation with three different examples
was identified during this period
concerning
Units
1
and 2,
and
a Notice of Violation (Appendix A) is attached.
This violation concerns failure of your staff to take timely and effective cor-
rective actions to prevent recurring violations of 10 CFR 21 reporting require-
ments,
high radiation
area
gates
being left
unlocked
and
missed
firewatch
patrols.
We are particularly concerned
about
these
recurring problems in view
of .the broader corrective
actions
outlined in your Restart
Action Plan.
This
violation
has
been
categorized
by severity level
according
to
Appendix
C (Enforcement Policy).
You are required to respond to this violation
and in preparing your response
you should follow the instructions in Appendix A.
The response
required
by this letter is not subject to the clearance
procedures
of the Office of Management
and
Budget
as
required
by the
Paperwork
Reduction
Act of 1980,
