This letter transmits
the results of the routine inspection
conducted
by
Messrs.
W. Cook,
R. Laura,
R.
Temps
and others of this office on November
18,
1988 through January
6,
1989 at Nine Mile Point Units
1 and 2, Scriba,
New
York.
A summary of the findings was presented
to Mr.
R. Abbott and other
members of your staff at the conclusion of the inspection.
During this period the inspectors
noted three different errors
made during the
work release
process
in Unit 2.
Two errors were related
to the tagout
system,
and the other
was the result of maintenance
being performed
on the wrong
component.
Although individually these
events
are of minor safety
significance,
we are concerned
that they indicate
a downward trend in
performance,
specifically inattention to detail.
At the exit meeting, your
staff acknowledged
our concern
and stated corrective action
was already in
progress
to reverse this trend.
We intend to closely monitor this recent
trend in performance at Unit 2 and discuss
our observations
with you during
our routine communications.
The inspectors
also identified an unsatisfactory
condition in the Unit 2
service water bays involving uncontroled
leakage of service water spilling on
equipment
and electrical
components
in the space.
We are particularly
concerned
that this condition apparently existed for sometime without
appropriate attention or corrective action by your staff.
In addition,
as discussed
in section 7.e. of the enclosed
report,
please
respond
to this office confirming your intentions in resolving the Unit
1
vi'tal equipment
access
controls
and your tentative
schedule
for completion of
this effort.
The response
requested
by this letter is not subject to the clearance
procedures
of the Office of Management
and Budget as required
by the Paperwork
Reduction Act of- 1980,
