This letter transmits
the results of the routine inspection
conducted
by
Messrs.
W. Cook,
W. Schmidt and
R.
Temps of this office on October 4,
1988
thru November
17,
1988 at Nine Mile Point Units
1 and 2, Scriba,
A
summary of the-findings was presented
to Mr. K. Dahlberg
and other members of
your staff at the conclusion of the inspection.
The recent
increase
in the
number of personnel
errors during the performance
of procedures
is an area of increased
interest
and concern to the
NRC.
Although
some of the errors clearly have mitigating circumstances,
several
have
been
caused
by the failure of personnel
to follow written procedures.
The NRC's concern
in this area
has
been
communicated to your senior site
management.
The resident
inspectors will be closely monitoring this trend in
personnel
errors to ascertain
management
effectiveness
in improving the situation.
In addition, Section
5 of the enclosed
report concluded that your staff's
preparation
and execution of maintenance
repairs to the recirculation
loop
discharge
valve were well thought out and well performed.
However,
a concern
remains regarding your failure to determine
the root cause for the binding.
Please
keep our inspectors
informed of your plans in this regard.
During followup on
NRC Temporary Instruction (TI) 2515/98,
concern
over high
temperatures
in the Unit
1 drywell and the effect on environmental qualification
of electrical
components
in the area
were identified and brought to your staff's
attention for review and analysis.
A Notice of Violation, issued
in Inspection
Report 50-220/88-17,
regarding
Technical Specification
(TS) violation of firewatch requirements
is withdrawn
following review of your response
letter, dated October
17,
1988.
The
NRC
agrees with your contention that since the fire door was not breached
during

the time in question
the
TS was not violated.
However,
we maintain,
and
your staff has agreed,
that further enhancement
in your administrative
program
concerning
issuance
and placement of breach permits is necessary.
This will
be reviewed during
a 'future routine inspection.
