APPENDIX A NOTICE OF VIOLATION Niagara Mohawk Power Corporation Nine Mile Point Nuclear Station, Units 1 and 2

Docket No. 50-220 License No.

DPR-63 As a result of the inspection conducted August 24-27,

1987, and in accordance with the "General Statement of Policy and Procedure for NRC Enforcement Actions",

10 CFR Part 2, Appendix C (Enforcement Policy, 1986), the following violation was identified:

Technical Specification

6. 11 requires that procedures for radiation protection be prepared and adhered to for'all operations involving personnel radiation exposure.

Radiation Protection Procedure S-RTP-10

requires, in part, in section 5.4.3 that a technician or supervisor will complete the whole body count worksheet and calculate the quantity of intake when an individual's whole body count result exceeds the Action Points presented in Table 12 of the procedure.

Contrary to the above, an individual's whole body count exceeded the Table 12 Action Points on March 17, 1986 and March 19, 1987 and the individual's intake was not calculated.

This-is a Severity Level V violation (Supplement IV)

Pursuant to the provisions of 10 CFR 2.201, Niagara Mohawk Power Corporation is hereby required to submit to this office within thirty days of the date of the letter which transmitted this Notice, a written statement or explanation in reply, including:

(1) the corrective steps which have been takin and the results achieved;

('2) corrective steps which will be taken to avoid furthe'r violations; and (3) the date when full compliance will be achieved.

Where good cause is shown, consideration will be given to extending this response time..
