This refers to an Operational
Readiness
Team inspection
conducted
by Mr. J.
Johnson
and other representatives
of the
NRC on June
1,
1987 to June
12,
1987
at Nine Mile Point Unit 2, Scriba,
A summary of the findings was
presented
to Mr. T. Perkins
and other members of your staff periodically during
the inspection
and to you at the conclusion of the inspection.
A violation of
NRC regulations
involving management
control of feedwater
system valve oper-
ations
and testing
was identified during the inspection.
You are required to
respond to this violation as described
in Appendix A to thi's letter.
Overall, the inspection
team found that licensed activities were controlled in
an acceptable
manner.
.The operators
were knowledgeable
and exhibited
a strong
sense
of responsibility.
However,
the
team found that site
management
was
sometimes
slow in identifying and correcting onsite
problems.
The team also
identified weaknesses
in training programs,
and certain other weak policies
and practices.
In addition,
an unresolved
item regarding
exemption
from
certain licensed
operator requalification classes
was identified.
In your
response
to this report,
please
include those actions you have taken or plan
to take regarding the, weaknesses
summarized
in Sections
1.2 and 6. 1 of the
attached
report.
Since it is possible that these
weaknesses
indicate
a declining performance
trend, I suggest that you carefully evaluate
them.
I would like to meet with
you to discuss your evaluation of these findings and your self-assessment
of
plant operations
during the power ascension
test program.
The responses
requested
by this letter and the accompanying
Notice are not
subject to the clearance
procedures
of the Office of Management
and Budget
as required by the Paperwork
Reduction Act of 1980,
