APPENDIX A NOTICE OF VIOLATION Niagara Mohawk Power Corporation Nine Mile 2 Docket No.

50-410 License No.

CPPR-112 As a result of the inspection conducted on August 11-15, 1986 and in accordance with the NRC Enforcement Policy (10 CFR 2, Appendix C), the following violation was identified:

10 CFR 50, Appendix B, Criterion V, requires that "Activities effecting quality shall be prescribed by documented instructions, procedures....

and shall be accomplished in accordance with these instructions, procedures..."

Nine Mile Point Unit 2, procedure N2-IMP-EQM-9005, Rev. 0, "Environmental Quali-fication Requirements for Maintenance and Replacement of Rosemount Transmitters" requires that Rosemount transmitter covers be torque tightened to assure adequate environmental seal and proper operation in a harsh environment.

Contrary-to the above, the licensee failed to follow this procedure in that on August 14, 1986 two safety related Rosemount transmitters, 2RHS*PT114 and CSH"PT105, included in the above procedure, were identified with loose covers.

Further, procedures were not provided which specified guidance for restoring Rosemount transmitters to qualified status in that Deficiency Report ¹17783 does not provide instructions to replace ethylene propylene O-rings, use Dow Corning 55MM grease nor give torque requirements.

This is a Severity Level IV violation.

Pursuant to the provisions of 10 CFR 2.201, Nine Mile-2 Power Station is hereby required to submit to this office within thirty days of the date of the letter which transmitted this Notice, a written statement or explanation in reply, including: (1) the corrective steps which have been taken and the results achieved; (2) corrective steps which will be taken to avoid further violations; and (3) the date when full compliance will be achieved.

Where good cause is shown, consideration will be given to extending this response time.
