APPENDIX A NOTICE OF VIOLATION Niagara Mohawk Power Corporation Nine Mile Point Unit 2 Docket No.

CPPR-112 As a result of the inspection conducted on February 22, 1986 to April 18, 1986 and in accordance with 10 CFR 2, Appendix C (1985), the following violation was identified.

This is a Severity Level IV Violation (Supplement II).
Pursuant to the provisions of 10 CFR 2.201, Niagara Mohawk Power Corporation is hereby required to submit to this office within 30 days of the date of the letter transmitting this Notice, a written statement of explanation in reply to include: (1) the corrective steps which have been taken and the results achieved; (2) the corrective steps which will be taken to avoid further vio-lations; and (3) the date when full compliance will be achieved.
Where good cause is shown, consideration will be given to extending time.
10 CFR 50, Appendix B Criterion III requires that the system design basis shall be correctly translated into design drawings.
The Institute of Electrical and Electronics Engineers (IEEE) Standards 308 and 384 specify that Class IE electrical equipment be protected from damage during design basis events.
Contrary to the above, as of March 25,
- 1986, the system design basis was not correctly translated into the design drawing in that Class IE conduits 2CX164GA and 2CX192YA were routed in the non-Category I Reactor Building stair tower in accordance with design drawings EE-460-BE and EE-460-BC and were therefore exposed to potential dama e due to ossible colla se of the stair tower during OFFICIAL RECORD COPY PDR AOOC '60508 8605190276.
