The inspection
was conducted
by
the
NRC's Office of Inspection
and Enforcement for the
EA Technical Audit and
by
NRC Region .I for the guality Assurance Audit.
The team was
composed of
personnel
from NRC's Office of Inspection
and Enforcement,
a representative
from Region I, and consultants.
The inspection
took place at the Stone
and
~
Webster Engineering
Corporation
(SWEC) Boston, Massachusetts
from April 22,
1985 to April 23,
1985.
The NRC's inspection of the proposed
EA Technical Audit of Nine Mile Point
2
was organized into the five major disciplines involved; viz, mechanical
systems/
power engineering,
engineering
mechanics, electrical,
instrumentation
and
controls
and structural.
In each of these
areas,
the
NRC inspectors
concentrated
on the preparations
of SWEC review plans to implement the proposed
EA Technical
Audit.
Where warranted,
the
NRC reviewed the resumes
and certifications of the
NMPC/SWEC audit teams.
The
NRC inspection
team
had the following general
comments
on the
EA Technical
Audit review plans that were inspected:
A number of .review plans either did not contain sufficient 'detail for
the
NRC inspectors
to gauge the depth of review to be performed
by
SWEC or in some
cases
the review plans
were not available.
In most
of these
instances,
discussions
with the reviewers
revealed that
sufficiently in-depth reviews would be conducted.

There were
no coordinated
review plans for reviewing the high-energy-
line break area
or the Seismic II/I area.
The Reactor
Core Isolation Cooling (RCIC) system
may not lend itself
to a complete review of the design process
used to evaluate
safe
shutdown capability following a high-energy-line
break.
Review of Seismic II/I considerations
by a plant walkdown technique
may not reveal
enough information on the design process
relative to
this issue.
Since
no cut-off date
has
been established
relative to document
review, consideration
should be given to whether revisions to design
documents
made since selection of the
RCIC impact the system
as being
a representative
sample of the technical
adequacy of the
NMP-2 design
and of the functioning of the design process.
The
NRC inspection
team understands
that the following items are included in
the electrical"discipline audit:
Electrical protection of motors
and motor operators
DC motor-operated-valve
voltage drop 120
V AC and
Control circuit voltage drop
Containment electrical penetration protection
Motor starting voltage
when loading diesel
generators
Instrumentation
and control power supplies
Specific comments applicable to the various technical disciplines of the
Technical Audit are provided in the enclosure
to this inspection report.
The
NRC's inspection of the proposed
SWEC gA Audit noted that detailed audit
. plans will be developed for each functional area of the audit.
However, the
NMPC/SWEC gA audit team only had the
RCIC instrumentation audit plan complete
and that plan was inspected
by the team.
The team questioned
Section 3.1 of
the
gA audit plan which specifies that attributive checks will be developed
to
assess if achievement
of instruction is satisfied.
This approach
allows the
gA
auditor to make judgments related to design intent and gives the auditor
the
authority to accept deviations
from criteria "as-is".
Therefore it is possible
that
some instances
of failure to construct in accordance
with design output
criteria will not be documented
during the audit and would distort'the results
of the "as built" review.
Further, it bypasses
the engineering organization
in making the decision of fit-for-use.
The reviewer discussed
this with the
gA audit team leader
and
SWEC upper
management.
We understand
that this will
be reconsidered
in the finalization of the audit plans.

covered in this inspection
plan will be reviewed
by
NRC during the
Implementation
Review at Cher ry Hill Operations
Center
scheduled for May
21-24,
1985.
