APPENDIX A NOTICE OF YIOLATION Niagar a Mohawk Power Corpor ation Docket No. 50-410 This refers to the inspection conducted by representatives of the Region I (Philadelphia) office of the Nine Mile Point Nuclear Station, Unit 2, Scriba, New York, of activities authorized by NRC License No. CPPR-112.
During this inspection, conducted on May 14-17, 1979, the following apparent item of noncompliance was identified:
10 CFR 50, Appendix B, Criterion V, states in part, "Activities affecting quality shall be prescribed by documented instructions, procedures, or drawings."
The Nine Mile Point PSAR, Appendix D, guality Assurance
- Program, Paragraph D.2.2, states, in part, that:
"NMPC will require Stone 8
Webster Engineering Corporation... to maintain and implement separate but Subsidiary guality Assurance Programs responsive to and designed to satisfy the intent of the guality Assurance Criteria of Appendix B."
Stone and Webster Engineering Assur ance Procedure EAP 16.2, Revision 2
establishes the responsibility for the Engineering, Construction or guality Assurance Department to review deficiencies in order to determine if they meet the criteria of 10 CFR 50.55(e).
Paragraph 4.8 of EAP 16.2 stated, in part, "If the... Department determines that a
deficiency ma be re ortable, the Project Manager shall immediately notify the client...
'ontrary to the above, as of May 15, 1979, there was no evidence that deficiencies in procedures and techniques for ultrasonic examination of double bevel "K" groove weld joints had been reviewed to determine if they meet the criteria of 10 CFR 50.55(e) although these deficiencies had been identified during a Stone 5 Webster audit in September, 1978 and on Noncompliance and Disposition Reports No. 1357, 1359 (both dated April 4, 1979) and 1380 (dated May 9, 1979).
