NRC Inspection Report Nos. 50-275/97-01 and 50-323/97-01, dated May 29,

1997, included one Severity Level IVviolation with two examples of failure to

implement procedures

in accordance with Technical Specification,

Administrative Controls, Section 6.8.1a.

The examples include failure to

provide operators with special prescription framed glasses for use with self-

contained breathing apparatuses

(SCBAs) and failure to complete annual

SCBA refresher training.

REPLY TO A NOTICE OF VIOLATIONIN

INSPECTION REPORT NOS. 50-275/97-01 AND 50-323/97-01

On May, 29, 1997, as part of NRC Inspection Report (IR) Nos. 50-275/97-01 and

50-323/97-01, NRC Region IV issued a notice of violation to Diablo Canyon

Power Plant, Units 1 and 2. The statement of violation and PGKE's reply are

documented below.

STATEMENT OF VIOLATION

Technical Specification, Section 6.8. 1a, requires, in part, thaf written

procedures be implemented covering the applicable areas recommended

in Appendix A ofRegulatory Guide 1.33, Revision 2, February 1978.

Appendix A of Regulatory Guide 1.33 lists activities fhat should be

covered by written procedures to include those for respiratory profection.

Procedure OM14./D2, "Medical Examinations," Revision 1A, requires that,

"individuals with prescripfion eyeglasses

who are required to wear a full-

face respirator shall use special frames for their glasses that do not

interfere with the face-piece seal." Further, Section 4.2.9b of Procedure

RP1./D3, "Respiratory Protection Program," states that, "onlyspecial

specfacle kifs specified by the manufacturer offhe respirator shall be used

so that the NIOSH approvalis not violafed."

Procedure TQ1.DC20, Revision 1, sfates, in part, that a two-part program

is used toinstruct personnelin the proper use and care ofrespiratory

profective equipmenf.

Inifialtraining program includes the proper actions

for abnormal or emergency sifuafions and a followup annual refresher

training checks the proficiency in the proper actions forabnormal or

emergency sifuafions.

Contrary to fhe above,

On 21 April 1997, it was identified that 26 licensed operators wifh

licenses conditioned forcorrective lenses did nof have special

prescription framed glasses for use wifh self-confained breafhing

apparafuses'available

in the control room for emergency use.

On 23 April 1997, it wasidentified that approximately 75 percent of

control room watchstanders had nof completed the annual refresher

associated

with the proficiency training in the use ofself-contained

breafhing apparatuses.

The above examples represent a Severity Level IV violation (Supplement

I) (50-275;-323/9701 -01).

PG8E agrees with the violation as stated in the IR.

Example

SCBAs.available in the control room for emergency use.

PG8 E attributed the condition to inadequate procedures and training. Neither

explicitly identified the responsibility to procure and maintain special prescription

frames for respirators or SCBAs.

Example 2 identified that approximately 75 percent of control room

watchstanders

had not completed the annual refresher associated with the

proficiency training in the use of SCBAs.

PG8 E attributed this condition to an inadequate procedure.

Although the

procedure required that the access supervisor maintain the most current security

level and radiological access category information in the Plant Information

Management System (PIMS), it did not specify the responsibility for maintaining

the respiratory protection access category.

Consequently, repiratory protection

training codes were not properly maintained for the operators.

Since the codes

were incorrect, the training notifications were not made.

CORRECTIVE STEPS TAKEN AND RESULTS ACHIEVED

tion Framed Glasses for Use With SCBAs

A prompt evaluation was done to assess

the operator's ability to carry out

licensed duties while wearing a SCBA using regular frames.

Based on

evaluation, the operators were informed that for SCBA use only, ifcorrective

lenses are needed to perform licensed duties, and special frames are not

available to wear within the face mask, the operator should wear the SCBA with

their regular glasses

in an emergency.

Operators received immediate practical training on SCBAs to fulfillthe annual

refresher training requirement.

During the training it was noted that all operators

were fullycapable of donning the SCBA correctly.

PIMS was changed to provide the proper training code for the operators and

members of the fire brigade.

This code requires operators and fire brigade

members to attend SCBA initial and refresher training, and respirator initial and

requalification training.

CORRECTIVE ACTIONS TO BE TAKENTO PREVENT RECURRENCE

Licensed operators and fire brigade members will receive special frames from

the Safety and Medical Services Section.

The frames will be maintained in a

designated storage area.

Other departments with personnel who may be required to wear special frames

with respirators or SCBAs in an emergency situation, willevaluate the need to

provide prescriptive frames.

The procedures and training curriculum for the Respirator Protection Program

will be revised to clearly indicate the joint responsibilities for procuring and

maintaining special prescription frames.

Com letion of Annual SCBA Refresher Trainin

Other departments with personnel designated for respirator and SCBA training

willverify the assigned codes are correct and make appropriate changes.

The procedure for operator requalification willbe changed to ensure SCBA

refresher training is scheduled annually as part of operator requalification.

The procedure for controlling access codes will be revised to clearly identify the

joint responsibilities for developing access codes, applying the codes to

individuals, and making changes to approved codes.

actions by other departments will be completed by 09/01/97.

Licensed operators and fire brigade members will receive special frames from

the Safety and Medical Services Section by 10/01/97 by which date PGRE will

be in full compliance.
