This refers
to the inspection
conducted
by M. D. Tschiltz
and others of this
office on January
21 through March 2,
1996, at the Diablo Canyon Nuclear
Power
Plant,
Units
1
and
2, facility.
The purpose of the inspection
was to
determine
whether activities authorized
by the license
were conducted
safely
and in accordance
with NRC requirements.
At the conclusion of the inspection,
the findings were discussed
with you and those
members of your staff
identified in the enclosed
report.
Areas
examined during the inspection
are identified in the report.
Within
these
areas,
the inspection consisted
of selective
examinations
of procedures
and representative
records,
interviews with personnel,
and observation of
act>v>ties
in progress.
The results of this inspection
are
documented
on
pages
3 and 4, in the ehclosed
report.
Based
on the results of this inspection,
the
NRC has determined
that three
violations of NRC requirements
occurred.
The violations are cited in the
enclosed
Notice of Violation (Notice)
and the circumstances
related
to them
are described
in detail
in the subject
inspection report.
The first violation
involved the failure to adequately
perform the monthly channel
checks
on
postaccident
monitoring instrumentation.
The last
two violations involve the
failure to take prompt
and comprehensive
corrective actions for identified
conditioJIs adverse
to quality.
In one instance,
fire brigade
members
were not
adequately
trained;
in the
second
instance p the centri,fugal
charging
pumps
=,~
were placed
in an unanalyzed
condition during testing lineups.
This area is
of concern
since other inspection findings,
as well
as your own self-
assessments,
have identified deficiencies
associated
with your corrective
action program.
You are required to respond
to this letter
and should follow the instructions
specified
in the enclosed
Notice when preparing your response.
In your
response.
you should document
the specific actions
taken
and
any additional
actions
you plan to prevent recurrence.
Your response
may reference
or
include previous
docketed
correspondence, if the correspondence
adequately
addresses
the required
response.
After reviewing your response
to this
Notice, including your proposed corrective actions
and
the results of future
