APPENDIX A NOTICE OF VIOLATION Pacific Gas and Electric Company Diablo Canyon Nuclear Power Plant Docket:

50-275 License:

DPR-80 During an NRC inspection conducted on September 1 through October 15,

1994, one violation of NRC requirements was identified.

In accordance with the "General Statement of Policy and Procedure for NRC Enforcement Actions,"

10 CFR Part 2, Appendix C, the violation is listed below:

A.

Diablo Canyon Technical Specification 6.8. 1 states, in part, that written procedures shall be established, implemented, and maintained covering the applicable procedures of Regulatory Guide 1.33, Revision 2, dated February 1978.

Appendix A of Regulatory Guide 1.33, Revision 2,

recommends procedures covering surveillance testing; preventive maintenance; and startup, operation, and shutdown of safety-related systems.

1..2, "Alignment Verification Checklist," of plant Procedure OP H-5: II, Revision 8, "Control Room Ventilation Sy'tem Alignment Verification," required Breaker PY-11-18 to be closed.
2.. 1, "IY-ll, TRY-11, PY-11 and PY-llA Alignment Verification Checklist," of plant Procedure OP J-10: II, Revision 11, "Instrument AC System Alignment Verification,"

required all Breaker PY-ll individual load breakers be either closed or open and tagged by an active clearance request.

Contrary to the above, on September 22,

1994, Breaker PY-11-18, which provides an alternate source of power to Control Room Ventilation Transfer Switch
EJPA, was found in the open position without being tagged by an active clearance request.

This is a Severity Level IV violation (Supplement

1) (275/9424-01).

During this inspection, it was determined that a written response to the violation discussed above is not required.

This determination was based on the actions taken by the licensee to determine the root cause and implement the appropriate corrective actions.
