APPENDIX A NOTICE OF VIOLATION Pacific Gas and Electric Company Diablo Canyon Unit 1

Docket No. 50-275 License No.

DPR-80 During an NRC inspection conducted from April 24 through June 4 1994, a

violation of NRC requirements was identified.

In accordance with the "General Statement of Policy and Procedure for NRC Enforcement Actions,"

10 CFR Part 2, Appendix C, the violation is listed below:

Diablo Canyon Technical Specification 6.8.1 states, in part, that written procedures shall be established, implemented, and maintained covering the applicable procedures recommended in Appendix A of Regulatory Guide 1.33, Revision 2, dated February 1978.

Appendix A of Regulatory Guide 1.33, Revi~ion 2, recommends that procedures for preforming maintenance that can affect the performance of safety-related equipment be properly preplanned and performed in accordance with written procedures, documented instructions, or drawings appropriate to the circumstances.

Procedure DCP-111, Revision 3, "Thermal Insulation," Step 4.2. 1, states, in part, that

" Insulation materials other than stainless steel jacketed calcium silicate or reflective metal shall not be used inside the Containment without prior written NES authorization."

Contrary to this requirement, on Hay 5, 1994, while Unit 1 was in Node 3 and containment systems were required to be operable, aluminum tape had been used in several locations on reactor coolant system piping insulation without NES authorization.

This was an addition to the aluminum inventory in containment which was not accounted for.

Further, the tape was not evaluated for its potential as a source of debris which could affect the containment recirculation sump screens.

This is a Severity Level IV violation (Supplement I) (no response required).
