VIOLATION)
This refers to the routine inspection
conducted
by H. Hiller, and the
'inspectors
identified in the enclosed
report, of this office on April 24
through June
4,
1994.
The inspection
included
a review of activities
authorized for your Diablo Canyon Nuclear Generating Station,
Units
1
and
2,
facility.
At the conclusion of the inspection,
the findings were discussed
with those
members of your staff identified in the enclosed report,
Areas
examined during this inspection
are identified in the report.
Within
these
areas,
the inspection consisted
of selective
examinations of procedures
and representative
records,
interviews with personnel,
and observation of
activities in progress.
Based
on the results of this inspection,
certain licensed activities appeared
to be in violation of NRC requirements,
as specified in the enclosed
Notice of
Violation (Notice).
After entry into Node
3 at the end of the Unit
1 outage,
an
NRC inspector identified your use of aluminum tape
on several
locations of
piping insulation in containment.
The aluminum tape
had
been installed
without appropriate
technical
review.
The design of containment
requires that
be controlled to reduce
the generation
of hydrogen during
a design
basis
event
and that items like tape
be controlled to ensure operability of
the recirculation
sump.
The tape
was promptly removed.
Later analysis
determined that sufficient margin
was available for both concerns
with the
tape installed.
We are concerned
that, despite
several
opportunities to identify and correct
this deficiency,
no one questioned this installation.
Thjs and other issues
~

discussed
in this report indicate
a less
than
adequate
questioning attitude.
Although other corrective actions
you took in response
to this finding were

prompt
and appropriate,
the loss of control leading to the use of aluminum
tape in containment
is significant.
The concerns
identified during this inspection period (the use of uncontrolled
aluminum tape in containment,
the isolation of the low temperature
overpressure
protection
system for 25 minutes,
and the inadvertent
pressurization
of the
RHR system)
indicate
a less
than
adequate
questioning
attitude
by your staff.
As we stated
in the forwarding letter of NRC
Inspection
Report 50-275/94-11;
50-323/94-11,
we remain concerned
about the
implementation of management
expectations:
the lack of sensitivity to the
need to pay close attention to detail, to fo11ow procedures,
and to not
proceed
in the face of uncertainty.
Because
we have already reviewed your corrective action~ for the enclosed
violation,
no further response
is required.
In accordance
with 10 CFR 2.790(a) of the NRC's "Rules of Practice,"
a copy of

in the
