SUBJECT: NRC INSPECTION REPORT 50-275/98-21; 50-323/98-21 AND NOTICE OF

VIOLATION

Dear Mr. Rueger:

From December 4 to 18, 1998, a special inspection was conducted at your Diablo Canyon

Nuclear Power Plant, Unit 2, reactor facility. The focus of the inspection was to evaluate

operator performance in response to the December 1, 1998, manual reactor trip and loss of

circulating water. The enclosed report presents the scope and results of that inspection.

Overall, operators responded satisfactorily to the reactor trip and loss of circulating water, the

normal heat sink. The operators effectively stabilized the plant in a safe condition. However,

based on the results of this inspection, the NRC has determined that one violation involving two

examples of failure to meet NRC requirements occurred.

The violation is cited in the enclosed

Notice of Violation (Notice) and the circumstances surrounding it are described in detail in the

subject inspection report.

In the first example, a misunderstanding by operators regarding the

effect of adjusting the atmospheric dump valve resulted in a high pressure setting that

unnecessarily challenged the main steam safety valves.

In the second example, a lack of

understanding of the response of the circulating water differential pressure indication to a

reactor trip resulted in a delay in securing the circulating water pump. These examples were

both contrary to procedure guidance.

We acknowledge that your staff likelywould have identified the first example during their

posttrip review, which, according to the Enforcement Manual, qualifies for licensee identification

credit. However, notwithstanding the guidance in Section VII.B.1 of the Enforcement Manual,

this example is being cited because both violation examples were partially caused by training

weaknesses

and because this example of the violation resulted in an unnecessary

liftof Main

Steam Safety Valve RV-7. In addition, the inspectors noted that a lack of formality and

structure in evaluating human performance and a lack of detail in operating logs made'it difficult

to reconstn;ct the event.

The NRC has concluded that information regarding the reason for the violation, the corrective

actions taken and planned to correct the violation and prevent recurrence, and the date when

full compliance was achieved is already adequately addressed

on the docket in the enclosed

inspection report. Therefore, you are not required to respond to this letter unless the

In that

case, or ifyou choose to provide additional information, you should followthe instructions
