(NOTICE OF
VIOLATION)
This refers to the routine inspection
conducted
by H. Miller, and the
inspectors
identified in the enclosed
report, of this office on February
17,
through March 19,
1994.
The inspection
included
a review of activities
authorized for your Diablo Canyon Nuclear
Power Generating Station,
Units
1
and 2, facility.
At the conclusion of the inspection,
the findings were
discussed
with those
members of your staff identified in the enclosed
report.
Areas
examined during this inspection
are identified in the report.
Within
these
areas,
the inspection consisted of selective
examinations
of procedures-
and representative
records,
interviews with personnel,
and observation of
activities in progress.

Based
on the results of this inspection,
certain licensed activities were in
violation of NRC requirements,
as specified in the enclosed
Notice of
Violation (Notice).
This violation is of concern
because it involved
a
failure to appropriately
implement the requirements
of an operating
procedure
during reduced
system inventory activities.
Operators
did not
properly vent the reactor vessel
head during
a draindown of the Unit
1 reactor
coolant system.
This resulted
in an abrupt
change
in
RCS level indication of
over six feet.
Although the reactor core
was never in any danger of becoming
uncovered during this event,
the event illustrates the need to positively
control operations
at reduced
inventory and the need for operators
to
anticipate the effects of their actions
before proceeding.
Regarding
an unrelated
issue,
while evaluating
an
NRC followup item during the
period of this inspection, it is our understanding
that Mr. M. Angus,
Manager,
Nuclear Engineering Services,
committed to install
ASME required tailpipe
'rains
in all relief valve tailpipes for which ASHE code
commitments
were

You are required to respond
to this letter and should follow the instructions
specified in the enclosed

actions you plan'to prevent recurrence.
After reviewing your response
to this
Notice, including your proposed corrective actions
and the results of future
inspections,
the
NRC will determine
whether further
NRC enforcement
action is
necessary
to ensure
compliance with NRC regulatory requirements.
In accordance
with 10 CFR 2.790(a) of the NRC's "Rules of Practice,"
a copy of
this letter
and its enclosure(s) will be placed in the
NRC Public Document
Room.
