This letter refers to the special

inspection

conducted

by Nr. L. Miller of

this office on February

11-14,

1991.

The inspection

included

a review of.

Mechanical

Maintenance activities in controlling measuring

and test equipment,

the results of evaluations of this area

performed

by your quality organizations,

and the adequacy of actions

taken

by your staff in resolving identified problems.

At the conclusion of the inspection,

the findings were discussed

with

Mr. J.

D. Townsend

and those other members of your staff identified in the

enclosed report.

Areas

examined during this inspection

are identified in the report.

Within

these

areas,

the inspection consisted

of selective

examinations of procedures

and quality records,

interviews with personnel,

and observation of activities

in progress.

Based

on the results of this inspection,

one apparent violation was identified.

This apparent violation is being considered for escalated

enforcement action in

accordance

with the "General

Statement

of Policy and Procedure for NRC

Enforcement Actions" (Enforcement Policy) 10 CFR Part 2, Appendix

C (1990).

The apparent violation concerns:

1) the chroni'c programmatic

weakness

in the

control of measuring

and test equipment maintained

by the Mechanical

Maintenance

Department;

2) the inability on the part of the guality oversight

groups to ensure

the deficiencies

were appropriately

addressed

by Mechanical

Maintenance in a timely, thorough manner;

and 3) the failure of management

to

insist,

and then follow through, to ensure corrective action was

implemented

to preclude recurrence.

This breakdown

was identified in April 1990 by your

guality Control department

during

a surveillance of measuring

and test

equipment controlled by your Mechanical

Maintenance

department.

Apparently,

no

effective corrective action was taken to address

this programmatic

breakdown

prior.to the

NRC inspection

documented

by NRC Inspection Report No. 50-275/90-29.

Because

escalated

enforcement action is being considered,

no Notice of Violation

is presently being issued for these

inspection findings.

In addition, please

be

advised that the

number

and characterization

of apparent violations described

in

the enclosed

inspection report may change

as

a result of further

been scheduled

for March 8, 1991.

The purposes

of this conference

are:

to discuss

the apparent

violation, its causes

and. safety 'significance; to provide you the opportunity

to point out any errors in our inspection report; to provide an opportunity for

you to present your proposed corrective actions;

and to discuss

any other

information that will help us determine the appropriate

enforcement action in

accordance

with the Enforcement Policy.

You will be advised

by separate

correspondence

of the results of our deliberations

on this matter.

No response

regarding the apparent violation is required at this time.

In accordance

with 10 CFR 2.790 of the

NRC's "Rules of Practice,"

a copy of this

letter and its enclosure will be placed in the

NRC Public Document

Room.
