This refers to the routine inspection

conducted

by Messrs,

P.

P.

Narbut and

K.

E. Johnston

during the period of March 11 through April 21,

1990.

This

inspection

examined your activities

as authorized

by

NRC License

Nos.

DPR-80

and

DPR-82.

At the conclusion of the inspection,

discussions

of their

findings were held with Mr. J.

D.

Townsend

and other members of your staff.

Areas

examined during this inspection

are described

in the enclosed

inspection

report.

Within these

areas,

the inspection consisted of selective

examinations

of procedures

and representative

records,'nterviews

with

person"el,

and observations, by the inspectors.

Based

on the results of this inspection, it appears

that one of your

activities was not conducted

in full compliance with NRC requirements,

as set

forth in the Notice of Violation, enclosed

herewith as Appendix A.

Your

response

to this notice is to be submitted in accordance

with the provisions

of 10 CFR 2.201

as stated

in Appendix A, Notice of Violation.

The enclosed

report describes

several

occurrences

where the significance or

scope of applicability of technical

issues

were apparently

not fully thought

out initially.

Examples include:

Failure to maintain the Fuel Handling Building Ventilation System barrier

doors closed

such that the system could establish

the required negative

pressure.

Engineering

reviews in 1989 recognized

the need to control the

system barrier, but then only considered

a limited number of openings in

the barrier.

Improper blowdown settings of several relief valves,

which were

identified during the Unit 1 outage in October to December,

1989, yet the

full extent of the issue

as it applied to other relief valves in the

plant was not determined until March,

1990.

Failure to evaluate .and correct

a problem identified with an Auxiliary

Feedwater

System

check valve during the second refueling outage of Unit

2.

During the third refueling outage,

the valve was found stuck open

during maintenance,

and then the problem subsequently

was fully

addressed.

Excessive

leakage

past three Unit 2 Residual

Heat

Removal

valves not

being fully assessed

when discovered

during the outage

and not being

promptly brought to the attention of management.

Problems with the operation of the Fuel Handling Building Brid~e Crane

during the Unit 2 outage,

which were not brought to management

s

attention

and were addressed

in a weakly coordinated

manner.

Although none of the above

examples

resulted in a significant safety issue,

these

examples

indicate that plant problems

are not always being fully and

promptly assessed.

- In accordance

with 10 CFR 2.790 (a),

a copy of this letter and the enclosures

will be placed in the

NRC Public Document

Room.

The responses

directed

by this letter are not subject to the clearance

procedures

of the Office of Management

and Budget as required

by the Paperwork

Reduction Act of 1980,

PL 96-511.
