This letter refers to the routine inspection
conducted
by Messrs
M. Cillis and
M.
K: TenBrook of this office on October
25 through
November
2, 1989, of
activities authorized
by NRC License
Nos.
telephone calls of November 7-8,
1989,
and to the discussions
of our findings
held by Messrs
Cillis and TenBrook with Mr.
M.
G. Crockett and other members of
your staff at the conclusion of the inspection.
Areas
examined during this inspection
are described
in the enclosed
inspection
report.
Mithin these
areas,
the inspection consisted
of selective
examinations
of procedures
and representative
records,
interviews with personnel,
and
observations
by the inspectors.
Based
on the results of this inspection, it appears
that one of your activities
was not conducted
in full compliance with NRC requirements,
as set forth in the
Notice of Violation enclosed
herewith
as Appendix A.
Your response
to this
Notice is to be submitted in accordance
with the provisions of 10 CFR 2.201,
as
stated
in Appendix A, Notice of Violation.
In addition to the violation identified in Appendix A, paragraph
3.H of the
Inspection
Report discusses
other problems
associated
with the Spent
Fuel
Pool
radiation monitor alarms.
These
problems
included conflicting alarm response
instructions to personnel,
lack of tai lboard meetings
as
recommended
by
procedures,
and inconsistent
Operator
response
to the Spent
Fuel
Pool radiation
monitor annunciator.
These
problems indicate
a lack of both effective
supervision
and prompt decisive action to correct
an abnormal situation.
In
addition to your specific response
to Appendix A, we request that you address
the root cause, of the problems described
in paragraph
3.H and what actions
you
are taking to correct deficiencies.

NRC Public Document
