conducted
by Messrs.,

E. Johnston
during the period, of September
10 through October 28, 1989,
and
November 20,
1989.
This inspection
examined your activities
as authorized
by
NRC License
Nos.
DPR-80 and
At the conclusion of the inspection,
discussions
of their findings were held with Mr. J.
D. Townsend,
and other
members of your staff.
Areas
examined during this inspection
are described
in the enclosed
inspection
report.
Within these
areas,
the inspection consisted of selective
examinations of procedures
and representative
records,
interviews with
personnel,
and observations
by the inspectors.
In the course of this inspection,
a number of examples
of management
oversight
concerns
were observed.
The examples
are listed in the "Results" section of
the report and discussed
in greater detail in the report body.
Two findings
highlight our concerns:
o
In an amendment
request to extend the allowed o'utage
time of the swing
diesel,
you provided erroneous
information about the ability to crosstie
diesel
generators
between units.
Plant personnel
discovered
the error
following the issuance, of the amendment,
at the
commencement
of the seven
.day swing diesel
outage.
On October 26, the inspector
found that numerous
maintenance
personnel
had been allowed to work more than
72 hours8.333333e-4 days <br />0.02 hours <br />1.190476e-4 weeks <br />2.7396e-5 months <br /> in a seven
day period without
the approval
or knowledge of plant management.
Additionally, following a
commitment to review the implementation of overtime limits, plant
management
failed to take aggressive
action, resulting in other
departments
exceeding
overtime limits.
These findings and others
described
in the report reenforce
a need for more
aggressive
and timely management
attention to problems.

on the results of this inspection, it appears
that
some of your
activities were not conducted
in full compliance with NRC requirements,
as set
forth in the Notice of Violation enclosed
herewith
as Appendix A.
Your

as stated in Appendix A, Notice of Violation.
