inspection
conducted
by Messrs.
P.
P. Narbut and
K.
E. Johnston
during the period of January
23 through March 17,
1989.
Additionally, this report includes
the results of a meeting held in the
NRC
Region
V Offices in Walnut Creek, California on April 6, 1989.
This
inspection
examined your activities
as authorized
by
NRC License
Nos.
and
At the conclusion of the inspection,
discussions
of our findings
were held with you and other members of your staff.
The inspection
included
an examination of multiple problems involving the
inoperability of the Auxiliary Feedwater
System experienced
in December
1988
through February
1989.
Additionally, this report includes
an evaluation of
your Licensee
Event Reports written in response
to the
NRC findings pursuant
to
a special
team inspection,
as reported in NRC inspection report
50-275/89-01.
The specific areas
examined during this inspection
are described
in the
enclosed
inspection report.
Within these areas,
the inspection consisted of
selective
examinations of procedures
and representative
records,
interviews
with personnel,
and observations
by the inspectors.
Based
on the results of this inspection, it appears
that certain of your
activities were not conducted
in full compliance with
NRC requirements.
These
violations
have reenforced
our concerns
related to design basis
maintenance
and engineering
work.
Additionally, the adequacy of management
and quality
organization
followup and corrective actions
has
been identified as
a
potential continuing problem area
from these
inspection results.
Enforcement
related to these matters will be the subject of separate
correspondence.
In accordance
with 10 CFR 2.790 (a),
a copy of this letter and the enclosure
wi 11
be placed in the
NRC Public Document
Room.
