during the period of December 4, 1988 through January
21,
1989.
This inspection
examined your activities
as authorized
by
NRC License
Nos.
DPR-80 and
At the conclusion of the inspection,
discussions
of their
findings- were held with Mr.
B. Giffin, and other members of your staff.
Areas
examined
during this inspection .are described
in the enclosed
inspection
report.
Within these
areas,
the inspection consisted of selective
examinations
of procedures
and representative
records,
interviews with
personnel,
and observations
by the inspectors.
No violations of NRC
requirements
were identified within the scope of this inspection.
Your attention is directed to two areas
described
in this inspection report
which deserve
increased
management
attention.
The areas
of long standing
and
increasing
NRC concern
are referenced
in the inspection
summary
and include:
(1)
a lack of thorough
and timely resolution of problem areas
as evidenced
by
continuing operational
errors
and (2) the lack of developing proper
questioning instincts in plant personnel
as evidenced
by the conduct of an
experiment
and
a system modification by backshift personnel
without
consultation of management
or engineering
personnel.
In accordance
with 10 CFR 2.790 (a),
a copy of this letter and the enclosures
will be placed in the
NRC Public Document
Room.
