meeting held with Mr. Maneatis, yourself,

members of your staff at the Region

V office on October

26,

1988.

Subjects

discussed

during the meeting are

summarized

in the enclosed

meeting report.

This meeting

was part of an ongoing quarterly discussion

between

PGRE and

NRC

.which has

been useful to establish

a rapport

and understanding

of regulatory

issues.

In sumary,

the report focused

on two issues

developed

in the

September

29,

1988 D'iablo Canyon Systematic

Assessment

of Licensee

Performance

(SALP) report,

and highlighted by recent events.

First, there

has generally

been

a lack of timely implementation .of lessons

learned

from industry

initiatives as well as your own operating experiences.'econd,

events

indicate that 'appropriate plant 'personnel

instincts

have not been established,

e.g., to recognize

when procedures

or instructions

are incomplete,

and to stop

activities until problems

are fully resolved.

A key element of both items

'discussed

was that of personnel

accountability.

Individuals. need to have

a

sense of responsibility for problems identified to assure

that corrective

actions

are adequately

implemented.

We acknowledge that you and your staff

recognize

the problems identified and are considering

methods

to address

the

concerns.

We look forward to future discussions

to status your progress

in

these

areas.

however, written comments

regarding

the meeting or SALP report may be provided within 30 days of this

SALP

management

meeting.
