Subject:
LICENSEE EVENT REPORT (LER) 2-88-010-00 Our in-office review of the subject LER concluded'that the information presented did not thoroughly document the event circumstances or your corrective actions as we understand them.
Specifically, the LER did not address the fact that; (1) an Action Request, that identified seismic trip instrumentation testing as a
potential cause of reactor trips, had been prepared about a year previous to the=,event, (2) a design change to annunciate the problem had been considered in a
'preliminary fashion but no further steps had been taken to implement the design
- change, and (3) you had established an Event Investigation Team to evaluate and make recommendations on resolution of "in-house" and industry problems.
This finding is particularly disturbing given our recent conclusions on the acceptability of LER 1-88-024 as documented in our Inspection Report 50-323/88-19 dated September 28, 1988; and the fact that previous to these findings, the Pacific Gas and Electric Company had been highly rated in LER quality.
Further-, this'ack of self criticism and introspective, self appraisal strongly suggests that you should examine your LER reporting process and evaluate the need for improvements.
I look forward to discussing your conclusions on this matter and would suggest that we include this topic at the Systematic Assessment of Licensee Performance management meeting on October 26, 1988.-
