Plant Tour

The inspectors

walked through various

areas of the plant on

a weekly basis

to observe activities in progress;

to inspect the general

state of clean-

liness,

housekeeping'nd

adherence

to fire protection rules; to check the

proper approval of "man on the line, caution

and clearance"

tags

on equip-

ment,

and to review with operation

personnel

the status of various

systems

in the plant.

The inspectors

noted that the status of the systems

and the housekeeping

appeared

consistent with construction activities.

The reactor cavity is

still being maintained

as

a clean area.

No items of noncompliance or deviations

were identified.

Procedure

Review

An inspector

reviewed the following new or revised procedures:

upon this review and review of preoperational

test results

the

inspector

ascertained

that the recirculation valves for the motor-driven

Auxiliary Feedwater

Pumps

(AFP) would be improperly positioned during

performance of OP D-l:I.

Specifically,

OP D-1: I directs

opening the

recirculation valves

whereas

preoperational

testing

has

demonstrated

that these

valves

should

be throttled to eliminate the

15 gallons per

minute extra recirculation flow.

The applicant

has

committed to revise

OP D-1: I and

OP K-10D (Sealed

Valve Checklist for the

AFW System).

Accordingly, this commitment is

an open item and will be

a prerequisite

for ascension

to full power (80-12-01).

No items of noncompliance

were identified.

Alle ation Related to Heat Trace Installation

On May 22,

1981,

inspectors

met with an individual in the

RV office who

expressed

concern

as to the adequacy of Thermon heat tracing at Diablo

Canyon.

The individual stated,

without supplying the information source,

that

he was told by an engineer,

employed in a consulting capacity

by

PGE E, that

PGE E had encountered

problems with Thermon heat trace

and

has

continued to utilize Thermon material.

The allegation

was not substantiated.

The inspectors

investigated

the allegation through discussions

with site

and corporate office engineering

personnel

and examination of representative

records of problems

encountered

and resolutions

made for those problems.

The inspectors

found that no evidence exists to indicate that

Thermon heat

tracing is defective.

It was observed that heat trace

problems

encountered

were generally

due

to an apparent

excessive

amount of trace

used in some applications

which

caused

degradation

of the wire insulation

due to excessive

heat.

These

problems

were corrected

by utilizing Thermon design

engineers

on site

to individually custom design heat trace

systems for Diablo Canyon.

Since

design completion

and installation, the licensee

has

had

no significant

repetition of previously encountered

application problems.

No items of noncompliance

were identified.

Permanent

plant instrumentation

was

used to record preoperational

test

data without consideration

given to the accuracy of the permanently

installed indicators

when evaluating the results of testing.

The

applicant

has reviewed

a total of 372 preoperational

test procedures

to determine if permanently installed instrumentation

was used to record

data

and then evaluate

the impact on results after taking into account

instrument inaccuracies.

Of these

372 procedures

74 required resolution

of the acceptance

criteria.

In all but

3 of the 74 procedures

the

applicant justified his resolution

by revising the acceptance

criteria

or citing surveillance testing satisfactorily performed

by the Nuclear

Power Operations

(NPO) Division subsequent

to preoperational

testing.

The applicant

has determined that

3 preoperational

test. addenda

need

to be written and performed

as

a result of his review.

An inspector

has reviewed the applicant's report

and portions of several

preoperational

tests evaluating his resolutions

and justifications

and

has

determined

the applicant's

response

to this open item as satisfactory for unit

1

only.

The applicant

will submit

a similar report

on evaluation of Unit 2

preoperational

testing in the future.

Accordingly this open item is

and this bulletin is considered

IE Bulletin 81-02, Failure of Gate Type Valves .to Close Against Differen-

tial Pressure

(Closed):

The licensee

has

performed

a review of all

valves supplied for the

NSSS at Diablo Canyon.

Diablo Canyon

has

no

valves affected

by the subject Bulletin and it is accordingly considered

closed.

No items of noncompliance or diviations were identified.
